Pharmacological properties

Nitrofurantoin – antimicrobial agent of the nitrofuran group, uroantiseptic.

Antimicrobial properties are due to the ability of nitrofurantoin to affect various enzyme systems of microorganisms. Nitrofurantoin, due to the creation of high concentrations in the urine, is especially effective in the treatment of diseases of the urinary tract.

Nitrofurantoin has a bacteriostatic effect (bactericidal drug at high concentrations).

The spectrum of antibacterial activity includes most microorganisms that cause urinary tract infections. Bacterial resistance to nitrofurantoin is rare.

Nitrofurantoin-sensitive microorganisms: Enterococci, Escherichia coli, Citrobacter spp., Streptococci group B, Staphylococcus aureus, Staphylococcus epidermidis, as well as those that rarely cause urinary tract infections: Salmonello pneumonia spp. moderately sensitive: Klebsiella pneumoniae, Enterobacter spp., Proteus spp., Providencia spp .; resistant to the drug: Pseudomonas aeruginosa, Serratia spp., Proteus mirabilis, Pseudomonas cepacia, Acinetobacter spp.

Nitrofurantoin is well absorbed in the gastrointestinal tract. Bioavailability is approximately 50% (food increases bioavailability). Plasma protein binding is 60-95%. Partially metabolized in the liver. 30-50% of the dose is rapidly excreted in the urine unchanged, which explains the bacteriostatic and bactericidal action of the drug in urinary tract infections. Effective therapeutic concentration of nitrofurantoin in blood and tissues is not achieved. The half-life is 0.5-1 hours. The high concentration of the drug in the urine lasts up to 12 hours. The maximum concentration in blood serum after reception of 100 mg of drug is reached in 30 minutes. In anuria or renal failure with creatinine clearance below 45 ml / min, the antibacterial concentration of nitrofurantoin in the urine is not achieved, but the incidence of side effects increases. Penetrates through the placental, blood-brain barrier, penetrates into breast milk. The half-life is 0.3-1 hours.

Bacterial infections of the urinary tract (pyelitis, pyelonephritis, cystitis, urethritis), including for the treatment of recurrences, as well as for the prevention of infection during urological operations, catheterization, cystoscopy.
