Description

Composition and form of release Reopoligliucin 
The main active ingredients of the drug are dextran and sodium chloride. Water for injection acts as an auxiliary component.
100 milliliters of the drug contains 10 grams of dextran-40 and 0.9 grams of sodium chloride.

Release form
It is produced in the form of an injection solution, poured into bottles with a volume of 200 or 400 milliliters.

Pharmacological properties
This drug belongs to the group of plasma substitutes, anti-shock, hemodynamic and detoxification drugs.
The specified drug substance (dextran) is a dextran solution, which is a polymer of glucose, dissolved in isotonic sodium chloride solution. It is used as a plasma substitute, improves the rheological properties of blood, reduces its viscosity, restores microcircular blood flow, and normalizes blood pressure.
It has a small relative molecular weight, which is close to the molecular weight of blood albumin, slowly penetrating through the walls of blood vessels. If it is introduced into the bloodstream, then this substance will continue to circulate in it for a long time.
In addition, dextran has a high osmotic pressure, which is 2.5 times higher than the osmotic pressure characteristic of proteins found in blood plasma. As a result, it tends to retain fluid in the bloodstream, producing a hemodynamic effect.
It is also able to quickly increase blood pressure in acute blood loss and maintain it at a sufficiently high level for a long time. Therefore, it is vital in case of significant blood loss.

Indications
This drug is used for the prevention and treatment of hypovolemic or distributive shock.

Also used for:

  • performing transplant operations;
  • vascular operations;
  • plastic surgery.

Also, the drug is added to the perfusion fluid in heart-lung machines during heart surgery.
With the rapid introduction of this drug, the volume of blood plasma can increase by an amount that is twice the volume of the injected blood product, since every 10 milliliters promotes the redistribution of 20-25 milliliters of fluid from the tissues into the bloodstream.

Contraindications
Not applicable if the patient has hypersensitivity (allergy) to one of the components that make up the drug.

Do not use when:

  • increased intracranial pressure with trauma to the skull;
  • hyperhydration, hypervolemia, thrombocytopenia;
  • kidney diseases, which are accompanied by oliguria, anuria;
  • decompensated cardiovascular insufficiency of 2-3 degrees.

Use with caution in liver diseases.

Application during pregnancy and lactation
Reopoligliucin can be used for the treatment of pregnant and lactating women.
However, it should be used with caution and only if there are vital indications, if a woman has toxicosis or nephropathy of pregnancy.

Method of administration and dosage
The drug Reopoligliucin is administered intravenously, drip. Before administration, the drug is heated to 35-37 ° C.
The dose and rate of administration is determined by the doctor on an individual basis.
In case of impaired capillary blood flow in various forms of shock, as well as in the postoperative period, the maximum daily dose for adults is 20 ml per 1 kg of body weight, and for children – 5-10, maximum – 15 ml per 1 kg of body weight.
In operations using artificial blood flow, add to the blood at the rate of 10-20 ml per 1 kg of patient weight to fill the oxygenator pump. Its concentration should not exceed 3%.
When administered, the drug should not be mixed with other drugs. If necessary, a rapid jet intravenous administration of the drug is possible, at the rate of 15 ml per 1 kg of the patient’s weight.
For patients with hemorrhagic stroke or traumatic brain injury, the drug is administered in a dosage of no more than 10-15 ml per 1 kg of the patient’s body!

Overdose
In case of overdose, hypervolemia, hypocoagulation may occur.

Symptomatic treatment is recommended.

Side effects
Therapy with this drug can cause:

  • an allergic reaction in the form of pruritus, Quincke’s edema, anaphylactic shock, angioedema, skin rashes, itching, increased sweating;
  • fluctuations in blood pressure, tachycardia, shortness of breath, edema;
  • nausea, vomiting, dry mouth, abdominal pain;
  • headache, dizziness, tremors;
  • an increase in the volume of urine separation (an increase in urine output), a decrease in the volume of urine, the viscosity of urine;
  • acrocyanosis, hyperemia, decreased platelet function;
  • general weakness, lower back pain, feeling short of breath, cramps.
