Sulbactomax (ceftriaxone sodium) 1000 mg/500 mg. 20 ml. №1 vial

Lower respiratory tract infections; acute bacterial otitis media; infections of soft tissues, skin; kidney and urinary tract infections; infections of bones, joints; septicemia; infections of the abdominal cavity (peritonitis, biliary tract and digestive tract infections); meningitis; gonorrhea; prevention of infections in surgery.

Composition and form of release
The main active ingredients are ceftriaxone (in the form of sodium ceftriaxone) and sulbactam (in the form of sodium sulbactam) in a 2: 1 ratio.
It is produced in the form of a combined powder for injection in a dosage of 1.5 grams (1000 mg sodium ceftriaxone / 500 mg sulbactam).

Pharmacological properties
This drug is a two-component antibacterial drug for systemic use, which consists of two active ingredients: ceftriaxone and sulbactam. The antibacterial component of the drug is ceftriaxone, which is a third-generation cephalosporin.

It is active in relatively sensitive microorganisms, which are in the stage of active multiplication, due to the property of inhibiting the biosynthesis of the mucopeptide of the cell membrane.
Sulbactam, in turn, enhances the effect of ceftriaxone.
This drug is used to treat diseases caused by pathogens that are sensitive to this combination of drugs (ceftriaxone with sulbactam), both in complex and in monotherapy. In particular, it is prescribed for the treatment of infections:

  • lower respiratory tract;
  • acute bacterial otitis media of the middle ear;
  • kidney and urinary tract (pyelonephritis, kidney abscess, cystitis, apostematous nephritis);
  • skin, soft tissues (abscesses, phlegmons, purulent-inflammatory diseases of pararectal tissue, gangrene of the extremities);
  • bones and joints (bacterial arthritis, osteomyelitis);
  • abdominal cavity (peritonitis, cholecystitis, cholangitis).

Also prescribed for the treatment of septicemia, meningitis, gonorrhea.
It can be used to prevent the development of postoperative complications during surgical operations.
Sulbactam has no real antibacterial activity, except for two microorganisms (Neisseriaceae and Acinetobacter). However, biochemical studies have shown that due to the various mechanisms of action of this substance, it has the ability to enhance the effect of ceftriaxone against a wide range of clinically significant microorganisms.
Do not use for allergic reactions to drugs of the cephalosporin series, including ceftriaxone and / or sulbactam.
It is contraindicated to use calcium-containing preparations for preparing an injection solution.
Contraindicated in renal or hepatic failure, gastrointestinal diseases. Also not used in the treatment of premature babies and full-term newborns under the age of 28 days from birth.
Application during pregnancy and lactation
There is no evidence that the use of this drug is absolutely harmless to pregnant women and the fetus. Therefore, it is permissible to prescribe it when carrying a child only when the possible benefits of using the drug are higher than the possible harm, especially in the first trimester.
During breastfeeding, the drug enters breast milk in some amount. As a result, lactation should be suspended during the treatment process.
Method of administration and dosage
Sulbactomax is administered in the form of intramuscular or intravenous injections, in a single dose for adults and children over 12 years of age, 1-2 grams (in terms of ceftriaxone) 1 time per day, every 24 hours.
In severe cases, the dosage can be increased to 4 mg (in terms of ceftriaxone).
Children’s dosage is calculated at the rate of 20-50 mg per 1 kg of body weight (no more than 50 mg per 1 kg and no more than 2 grams per day, in terms of ceftriaxone).
The duration of the course of treatment is determined by the attending physician, depending on the severity of the course of the disease. Treatment usually lasts 4 to 14 days.
The preparation of the solution for injection is carried out by qualified medical personnel in accordance with the requirements approved in the special medical literature.
In case of an overdose, an increase in side effects, the occurrence of seizures, can be expected.
Hemodialysis is ineffective. Treatment of symptomatology is recommended.
Side effects:

Side effects are usually mild and do not interfere with continuing treatment. Most often they appear as:

  • nausea, vomiting, pseudomembranous colitis, diarrhea (diarrhea);
  • skin rash, urticaria, Stevens-Johnson syndrome;
  • candidomycosis, mycoses, including genitals;
  • precipitates of calcium salt of ceftriaxone in the gallbladder, increasing the activity of hepatic transaminases;
  • pain and / or phlebitis at the injection site, muscle twitching;
  • neutropenia, eosinophilia, thrombocytopenia, hematocrit, decrease in hemoglobin;
  • urticaria, edema, itching.
